The CNS provides service providers who so wish with forms enabling them to indicate, for each service, the qualifications actually used to carry out these services in compliance with the minimum qualifications required.
These forms are accompanied by the technical data sheets available below, which contain explanations for each of the forms.
The forms are specific to each type of service provider (Continuous-stay aid and care establishments, Aid and care networks, Intermittent-stay aid and care establishments, Semi-stationary centres) and are available on request from the following address

In your request, please indicate the list of entities for which you would like to receive the form and the corresponding provider codes. The first two tabs of the form will be pre-filled by the CNS. Once you have completed the form, you can return it to controlesad.cns@secu.lu.
