Orthopedists-Shoemakers-Bandagers

Allocation of a service provider code

As a service provider approved by the CNS, I receive a service provider code, which must then appear on all documents used in my dealings with the health insurance system.

Request for a service provider code

The following documents must be provided by the orthopedist-shoemaker-bandager requesting the service provider code:

  • the "Fiche de renseignement sur l'établissement au Luxembourg"(information sheet on the establishment in Luxembourg)
  • a copy of the manager's identity card
  • a copy of the authorization from the Ministry of Economy
  • a copy of the registration on the craftsman's register (inscription sur le rôle artisanal)
  • a copy of the company's articles of association if it is not a sole proprietorship
  • a bank account identification form (relevé d'identité bancaire-RIB)

Documents should be sent in PDF format to gefo.cns@secu.lu.

Requests in JPEG and HTM formats cannot be opened and will therefore not be accepted.

Electronic data exchange

In certain agreements between the CNS and various healthcare professionals, the organization of the management, transmission, and storage of documents is provided for exclusively in digital and electronic form. These include agreements between the CNS and medical analysis and clinical biology laboratories, physical therapists, and dietitians. For healthcare professionals other than doctors and dentists, electronic data exchange is optional.

To participate in electronic data exchange, service providers must connect remotely to a secure area where they can deposit their request files and retrieve the response files deposited there by the CNS. To do this, service providers must:

  • Connect to the secure “HealthNet” network to access the CNS server
  • Be able to generate request files that comply with the specifications set out in the agreement and import the files returned by the CNS into their own database.

In order to ensure confidentiality and data protection, service providers must connect to HealthNet, a highly secure IT network provided by Agence eSanté. Service providers must then request a username and password to access the CNS server, which is also connected to HealthNet.

The relevant forms and user guides are available below. New providers wishing to log in for the first time are advised to consult the “Guide utilisateur pour activer une nouvelle connexion électronique avec la CNS"(User guide for activating a new electronic connection with the CNS) first.
