Accreditation and allocation of a service provider code

In order to provide services under the health insurance scheme with a taxi company, I must be accredited by the CNS and have been assigned a service provider code. Accreditation is granted by the Council of administration of the CNS upon presentation of the necessary documents and if all other conditions are met.

The conditions and necessary documents are listed under “Everything I need to know.”

Once approval has been granted, a service provider code is assigned and must appear on all documents used in dealings with the health insurance system.

Conditions for obtaining accreditation

Accreditation may be requested by any natural or legal person authorized to operate a taxi or chauffeur-driven car service. To do so, I must hold:

  • the establishment authorization required under Article 1 of the Law of September 2, 2011, regulating access to the professions of craftsman, merchant, industrialist, and certain liberal professions (hereinafter the Law of September 2, 2011) and
  • where applicable, a taxi operating license as provided for in Article 3 of the Law of July 5, 2016 on the organization of taxi services and amendment of the Consumer Code (hereinafter referred to as the “Law of July 5, 2016 on the organization of taxi services”).

Documents required for accreditation

  1. the accreditation application, duly signed, and the general terms and conditions and appendices, initialled on each page (CNS form);
  2. the “Fiche de renseignements sur l'établissement au Luxembourg" (Information sheet on the establishment in Luxembourg) (CNS form);
  3. a copy of your identity card or passport;
  4. a copy of the authorisation to practise issued by the Minister responsible for authorisations to establish a business;
  5. proof of affiliation to the Chamber of Skilled Trades and Crafts (Chambre des Métiers);
  6. a bank account identification form (relevé d'identité bancaire-RIB);
  7. a copy of both sides of all registration certificates for vehicles in service that may be used to transport persons covered by health insurance in connection with the coverage of transportation costs;
  8. a copy of the valid tax stickers issued by the Customs and Excise Agency (Administration des douanes et accises) for each vehicle;
  9. a copy of the roadworthiness certificate for vehicles in service that may be used to transport persons covered by health insurance in connection with the coverage of transport costs;
  10. the names and registration numbers of drivers who may provide transport;
  11. a certificate proving that, over the last three years, the signatory service provider has complied with the obligations to declare and pay advances and principal amounts on each due date, or that it has complied with the deadlines granted to it, as established by:
    • the Social Security Centre (Centre commun de la sécurité sociale) and
    • the Direct Taxation Administration (Administration des contributions directes) and
    • the Registration and Domains Administration (Administration de l'enregistrement et des domaines);
  12. the “Taxi/Ambulance Data Statement” (Relevé des données -Taxi/Ambulance) (CNS form).

Additional documents if the service provider is authorized to operate a taxi service

  1. a copy of the taxi operating license issued by the Minister responsible for transportation;
  2. a copy of the card of each driver who may provide transportation, issued by the Minister responsible for transportation.

Additional documents if it is a commercial company

  1. the company's articles of association, specifying the contact details and functions of the persons responsible, in particular the technical manager;
  2. an original extract from the trade and companies register dated within the last three months.

Documents to be provided in order to maintain accreditation

Accredited service provider - Documents to be submitted annually to the CNS

The following documents must be submitted by the service provider to the relevant department of the CNS by June 30 of each year, failing which accreditation will be suspended:

  • proof of affiliation to the Chamber of Skilled Trades and Crafts;
  • copies of valid tax stickers for each vehicle issued by the Customs and Excise Agency until June 30;
  • a copy of the technical inspection certificate for vehicles in service;
  • where applicable, a list of branches operated.

Changes concerning vehicles and drivers

In order for transport by taxi or rental car with driver to be covered by health insurance, the vehicle and driver must be correctly declared to the CNS.

In this context, and under penalty of suspension of accreditation, the service provider accredited by the CNS undertakes to send the CNS a written update at least once a year by June 30 and at the latest upon any change in the composition of its vehicle fleet or the identity of its drivers.
