By ticking this box, I am aware that, in accordance with Article 169 of the CNS's statutes, an early return to work before the presumed end of the incapacity for work, as certified by the prescribing physician on the medical certificate of incapacity for work, puts an end to the incapacity for work *

To declare my early return to work (before the end date indicated by my doctor on my certificate of incapacity for work), I fill in ALL the fields on the form and then click on PREVIEW. Then, on the summary page, I go through the information one last time and confirm by clicking SEND.

If I am long-term incapacitated for work and I have already received a summons from the Contrôle médical de la sécurité sociale (CMSS), I must complete the reply coupon on the back of the summons I received by post and return it to the address shown on the reply coupon.

The rules

If I return to work before the end of my certificate of incapacity for work, it is important that I inform the CNS by completing the form on this page.

I do not need to provide a new medical certificate from my doctor to return to work.

I should make the declaration below no earlier than two days before I actually return to work.
