I am aware that all mail will be sent to the official address and not to the address where I am/will be staying and that the authorised hours of absence (10am-12pm and 2pm-6pm) from the 6th day onwards still apply at the address of stay *

To declare my temporary address in the same country as my official country of residence, I fill in ALL the fields and then click on PREVIEW. Then, on the summary page, I go through the information one last time and confirm by clicking on SEND.

The rules

As part of my incapacity for work, I declare a temporary address if I am not staying at my official address registered with the CNS.

This administrative step is important to maintain effective communication with the CNS.

What do I need to do?

If I am staying away from home during my sick leave, I must notify the CNS of my temporary address using the form on this page.

If this address is already shown on my certificate of incapacity for work, there is no need to do this.

If I wish or need to travel/stay abroad during my sick leave, I must request prior authorisation.
