Cross-border workers
I work in Luxembourg and live in another country: I am therefore a cross-border commuter. As a cross-border insured, I am entitled to health benefits in Luxembourg under the same conditions as residents of the Grand Duchy. The same applies to members of my family.
However, there are procedures to be followed in my country of residence if I want to be entitled to healthcare there. In this section, I'll find out everything I need to know to guarantee my health cover, whether I live in France, Belgium or Germany.
If I work in an EU country but return to live in another country every day, or at least once a week, I am considered to be a cross-border worker under European legislation.

How do I register in Luxembourg?
If I work in Luxembourg, either for an employer or as a self-employed person, I must be affiliated to the Joint Social Security Centre (Centre commun de la sécurité sociale CCSS). This affiliation protects me against various social security risks, such as sickness, maternity, accidents at work, occupational diseases, old age, invalidity and long-term care.
I am an employee
My employer is responsible for registering me with the CCSS.
I am self-employed
It is up to me to register with the CCSS.
How do I register in France?
To be entitled to healthcare cover in France, I must register with a local health insurance fund.
In France, this is the Caisse Primaire d'Assurance Maladie (CPAM) responsible for my departement.
This registration is done using form S1/S072, a document issued by the CNS to open entitlements.
I live in the Grand-Est region
I live in one of the following départements: 08-Ardennes, 10-Aube, 51-Marne, 52-Haute Marne, 54-Meurthe et Moselle, 55-Meuse, 57-Moselle, 88-Vosges, 67-Bas-Rhin, 68-Haut-Rhin)
→ The S072 entitlement document is sent directly to my local CPAM. This will confirm my registration without, in principle, any further procedure being necessary.
If I am affiliated through a temporary employment agency, the certificate of entitlement is not issued automatically. The request must be made :
- Either in writing by the competent CPAM ;
- Or online using Form S1 to be completed by the CPAM.
I live in another French department
My S1 entitlement certificate will be sent automatically to my home address so that I can forward it to the CPAM in my department.
If I have not received the S1 form within 15 days of receiving the declaration of entry from the Joint Social Security Centre (CCSS), I should order it myself.
The S1/S072 form is a certificate of entitlement to health benefits issued by the country that collects my social security contributions. It allows me to register with my CPAM in France. This means that healthcare I receive there is covered as if I were insured there.
How do I cover my family members?
The members of my family can obtain benefits in the Grand Duchy of Luxembourg under the same conditions as those applied to people resident in the Grand Duchy.
As I live in France, French legislation determines who is a family member entitled to benefits.
To this end, my CPAM contacts the CNS electronically via EESSI (electronic exchange of social security information) to request the S072 equivalent to the digital version of the S1. The CNS will confirm registration by returning the S073 form.
I should therefore contact my CPAM to check that it has taken the necessary steps with the CNS to co-insure my family members.
How do I declare my incapacity for work?
In the event of illness or accident, I send the original of my certificate directly to the CNS before the end of the third working day of incapacity for work.
Good to know: to facilitate the processing of my case and ensure prompt payment of my financial compensation, I ask my doctor to write my 13-digit Luxembourg identification number on the certificate. Failing that, I can add it myself to the back of the document.
How are my medical expenses reimbursed?
My medical expenses incurred in my country of residence
They will be reimbursed by the local health insurance fund in accordance with the conditions, provisions, rates and tariffs applied locally.

The cost of treatment received outside Luxembourg and outside my country of residence varies depending on whether the treatment is urgent (or necessary) or scheduled.
Coverage of my unscheduled care
Urgent care provided in Switzerland, Norway, Iceland, Liechtenstein or a European Union country is covered on presentation of the European Health Insurance Card (or a replacement certificate) issued by my country of insurance.
Coverage of my scheduled treatment
Prior agreement from the CNS is required for scheduled inpatient treatment (with overnight stay) in Switzerland, Norway, Iceland, Liechtenstein or a European Union country.

In Germany, I must choose a health insurance fund (gesetzliche Krankenkasse) from among the many available, then send the form to the fund I chose.
This registration is carried out using form S1, a document issued by the CNS to establish entitlement.
Once I am registered with the CCSS, the S1 form is usually sent automatically to my home address so that I can present it to my health insurance fund in Germany.
If I have not received the S1 form within 15 days of receiving the declaration of entry from the Joint Social Security Centre (CCSS), I should order it myself.

How do I declare incapacity for work?
In the event of illness or accident, I send the original of my certificate directly to the CNS before the end of the third working day of incapacity for work.
To submit my incapacity for work to the CNS, I ask my German doctor to provide me with a printout of the electronic incapacity for work certificate in duplicate:
- One copy without diagnosis for my employer;
- A copy with a diagnosis, mentioning my 13-digit Luxembourg national identification number, which I send to the CNS no later than the 3rd day of my incapacity for work.
I don't forget to make a copy of the certificate for my personal file, if necessary!
Good to know: to facilitate the processing of my file and ensure prompt payment of my financial compensation, I ask my doctor to write my 13-digit Luxembourg identification number on the certificate. Failing that, I can add it myself to the back of the document.

The BL1 form is a certificate of entitlement to health benefits issued by the country that collects my social security contributions. It allows me to register with the fund (mutualité) of my choice in Belgium. This means that healthcare I receive there is covered as if I were insured there.
How do I cover my family members?
The members of my family can obtain benefits in the Grand Duchy of Luxembourg under the same conditions as those applied to people resident in the Grand Duchy.
As I live in Belgium, Belgian legislation determines who is a family member entitled to benefits.
To this end, my Belgian health insurance fund (mutualité) sends the CNS a certificate confirming that my family members are covered by my insurance.
I should therefore contact my mutualité to check that it has taken the necessary steps with the CNS to co-insure my family members.
