Affiliation
If I work in Luxembourg, either for an employer or as a self-employed person, I must be affiliated to the Centre commun de la sécurité sociale (CCSS).
This affiliation protects me against various social security risks, such as sickness, maternity, accidents at work, occupational diseases, old age, invalidity and dependency.
I work in Luxembourg and am therefore covered by Luxembourg health insurance. On this page, I discover:
- Mandatory affiliation: If I work in Luxembourg, I must be affiliated to the CCSS for comprehensive cover against various risks.
- Affiliation procedure: As an employee, my employer is responsible for this. If I am self-employed, I must register myself.

How do I register?
If I work in Luxembourg, either for an employer or as a self-employed person, I must be affiliated to the Centre commun de la sécurité sociale (CCSS). This affiliation protects me against various social security risks, such as sickness, maternity, accidents at work, occupational diseases, old age, invalidity and dependency.
I am an employee
My employer is responsible for registering me with the CCSS.
I am self-employed
It is up to me to register with the CCSS.
Confirmation of my affiliation
Once my affiliation has been registered by the CCSS, I will receive confirmation (information concernant une déclaration d'entrée) by post to my official address. This document contains my 13-digit national identification number.
Which health insurance fund is responsible for me?
My declaration of entry will state which health insurance fund is responsible for me:
- Caisse nationale de santé (CNS), for insured persons in the private sector (salaried and non-salaried employees such as the self-employed) and manual workers employed by the State.
- Caisse de maladie des fonctionnaires et employés publics (CMFEP), health insurance fund for civil servants and public employees working for the State.
- Caisse de maladie des fonctionnaires et employés communaux (CMFEC), health insurance fund for civil servants and employees working for communes.
- Entraide médicale de la CFL (EMCFL), health insurance fund for employees of the CFL.
What are the benefits of my affiliation?
By joining the social security scheme, my family and I can receive health and maternity insurance benefits and services via the responsible fund.
I also receive a social security card bearing my 13-digit national identification number. This card must be presented to healthcare providers.
I quote my national number when dealing with my health insurance fund.
How do I know if I'm no longer an affiliate?
When I am no longer affiliated, I usually receive a confirmation of termination of affiliation, called 'Information concernant une déclaration de sortie'.
The Centre commun de la sécurité sociale (CCSS) sends me this document.
Important: I must keep my address up to date.
What should I do when my affiliation ends?
If my affiliation ends and I do not immediately start a new activity that entitles me to reaffiliate, there are several options available to me to remain covered for healthcare.
I am a resident of Luxembourg
Residual coverage (Article 8 of the CNS statutes)
If I have been continuously affiliated in Luxembourg for at least six months (an interruption of less than eight days is accepted), I am entitled to healthcare coverage for:
- the current month, and
- the following three months,
provided that I have no other legal cover for the same risks during this period.
Please note: This coverage is temporary.
Co-insurance
I will check whether I can be affiliated as a co-insured via a family member (e.g. spouse, partner).
Voluntary affiliation
Under certain conditions, particularly if co-insurance is not possible, I can apply for voluntary affiliation with the Social Security Centre.
I am a non-resident (cross-border worker)
I live abroad and am no longer affiliated in Luxembourg (e.g. after the end of an employment contract).
What should I do? Contact your local health insurance fund (in France: CPAM, in Germany: Krankenkasse, in Belgium: mutualité) to examine the possibilities for affiliation in your country of residence.
Depending on my situation, there are several options available:
Affiliation in my country of residence
If I can be covered on a personal basis (e.g. job seeker), the local fund will register me.
Affiliation via co-insurance
If personal coverage is not possible, the local fund will check whether co-insurance is possible (e.g. via my spouse).
If my spouse works in Luxembourg, co-insurance can be arranged with the CNS. The fund in the country of residence will contact the CNS.
Temporary coverage under Article 8 of the CNS statutes
If no health insurance cover is available in my country of residence, the local fund can contact the CNS to request the application of Article 8 of the CNS statutes.
This right allows temporary coverage of healthcare after disaffiliation (current month + 3 months), provided that you have been registered without interruption (max. 7 days) for at least 6 months before the end of your affiliation.
Please note: This coverage is not a new affiliation but a temporary continuation of your entitlement to healthcare benefits after the end of your affiliation.
Voluntary affiliation in Luxembourg
In addition to the entitlement under Article 8, or if no other solution applies, I can apply for voluntary insurance with the Social Security Centre (CCSS) under certain conditions.
This option is subject to a fee and allows me to remain affiliated with the Luxembourg health insurance system.
