Affiliation

Anyone working in Luxembourg, whether employed or self-employed, must be affiliated by the Social Security Centre (Centre commun de la sécurité sociale-CCSS).

This affiliation provides protection against various risks related to social security, such as illness, maternity, occupational accidents and illnesses, old age, disability and long-term care.

When a self-employed person starts working in the Grand Duchy, they must immediately contact the Social Security Centre (Centre commun de la sécurité sociale-CCSS), which is the competent institution for social security affiliation.

Confirmation of Affiliation

Once the affiliation has been registered by the CCSS, the insured person will receive a confirmation (‘information concerning a declaration of entry’) by post at his official address. This document contains, in particular, the 13-digit national identification number and the competent health insurance fund.



Which health insurance fund is responsible?

The declaration of entry will mention the competent health insurance fund:

  • National Health Fund (Caisse nationale de santé-CNS): for private sector insured persons (employees and non-employees such as the self-employed) and state workers.
  • Health Insurance Fund for Civil Servants and Public Employees (Caisse de maladie des fonctionnaires et employés publics-CMFEP).
  • Health Insurance Fund for Communal Civil Servants and Employees (Caisse de maladie des fonctionnaires et employés communaux-CMFEC).
  • CFL Health Insurance Fund (Entraide médicale de la CFL-EMCFL).

Any employer wishing to post an employee abroad for work purposes must first inform the Social Security Centre of their intention.

Employer obligations regarding the declaration of incapacity for work

The employer is obliged to provide accurate information on the monthly declaration of incapacity for work. This includes not only the periods of incapacity for work in the previous month, but also the exact number of hours of absence from work due to incapacity during working hours. These declarations must be made throughout the employee's affiliation, and not only during the period of continued remuneration, but also beyond.

Importance of the declaration

  • For the Social Security Centre (CCSS)

The declaration of incapacity for work is crucial for the CCSS because it allows the automatic calculation of the reimbursement by the Employers' Mutual Insurance Fund of the continued remuneration by the employer.

Employers are required to continue paying their employees' salaries until the end of the calendar month in which the 77th day of incapacity for work falls, within a reference period of 18 consecutive months. This generally corresponds to a period of approximately 13 weeks.

During this period of continued payment, the Mutual Insurance Fund reimburses 80% of this payment to the employer via the CCSS.

  • Importance for the National Health Fund (CNS)
  • These declarations are also essential for the CNS, as they enable it to pay the insured person any subsequent financial benefits.
  • After the period of continued payment, the CNS takes over the payment of a financial benefit, after having informed the employer of the termination of its responsibilities in this regard.
