Exceptional exit request

I am asking the CNS to lift an exit restriction

To request an exceptional absence from home authorisation by the CNS (request to lift an exit restriction) during my long-term sick leave, I fill in ALL the fields and then click on PREVIEW. Then, on the summary page, I go through the information one last time and confirm by clicking SEND.

Authorisation for an exceptional exit does not allow you to travel or stay abroad. If you wish to travel abroad, you must complete an Application for prior authorisation to travel abroad during the period of incapacity for work.

The rules

I'm on long-term sick leave. I have read the various rules that I must follow during my sick leave.

If I am unable to work for a period of more than 6 consecutive weeks (=42 days), and I would exceptionally like to be away from my place of residence for a few hours outside authorised absence hours (10am-12pm and 2pm-6pm), I can send a request to the CNS for an exceptional discharge starting on the 43^rd day of sick leave.

All I have to do is fill in the form on this page and wait for the CNS to reply. I must submit the request at least 48 hours in advance and I will receive a reply (positive or negative) from the CNS as soon as possible.

An authorisation for an exceptional absence from home does not allow me to travel or stay abroad. If I wish to travel abroad, I must complete an Application for prior authorisation to travel/stay abroad during the period of incapacity for work.
