The file I send to the CNS using the form on this page must be clearly legible and correctly cropped, have a maximum size of 5 MB and be in PDF format.
I am only using this form to send my digital certificate of incapacity for work to the caisse.

The rules

I'm working and expecting a baby. To prepare for childbirth and welcome my baby in the best possible conditions, I'm applying for maternity leave, which consists of 8 weeks' prenatal leave and 12 weeks' postnatal leave.

To qualify for this leave, I must have been affiliated to the health and maternity insurance scheme for at least 6 months in the 12 months preceding the planned start of maternity leave.

To be valid, the medical certificate must be issued by a doctor (not a midwife) during the last 12 weeks of my pregnancy. A certificate drawn up too early cannot be accepted by the CNS.
