Since 2024, more and more doctors and dentists have been using Immediate Direct Payment (PID).
Digital Reimbursement is another method gaining ground amongst doctors: the doctor submits the invoice electronically, and the insured person can then submit their claim for reimbursement digitally to their health insurance.
In practical terms, how do these two methods of coverage for my medical expenses work?

What are the benefits of PID?
No more upfront payment!
With PID, I no longer have to make an upfront payment. The portion normally reimbursed by the CNS (88% for doctors) is paid directly to the doctor. All I have to do is pay my personal contribution (the remaining 12%) and any charges for personal convenience.
Good to know: For children and young people under the age of 18, I don't have to pay any personal contribution, as there is 100% coverage!

In practical terms, what is PID?
Immediate Direct Payment (PID) enables my doctor to receive the portion of the cost covered by health insurance directly from the CNS. The bank transfer from the CNS takes place instantly, enabling my doctor to receive the corresponding amount in their bank account in just a few seconds.
Any amount I am required to pay (my co-payment) is paid directly to the doctor by me.
Does PID apply only to those insured with the CNS?
PID applies to all insured persons covered by health and maternity insurance in Luxembourg (CNS, CMFEP, CMFEC, EMCFL).
When using PID coverage, what documents will I receive?
From my doctor
My doctor provides me with a PID invoice.
From the CNS
Following PID coverage, I receive the following from the CNS:
- either a statement of services provided via direct coverage (PID), submitted digitally after each consultation directly to my private account on MyGuichet.lu, if I haveactivated the eDelivery function;
- or a statement of services provided via direct coverage (PID) sent by post one month after the consultation, if I have not activated the eDelivery function on MyGuichet.lu.
How do I activate the eDelivery feature on MyGuichet.lu to receive my PID statement digitally?
How do I activate eDelivery? It's very simple:
- Log in to your personal account on MyGuichet.lu using your authentication method (LuxTrust, GouvID, eIDAS) by clicking here.
- Under 'My details > Health insurance fund > Activating services', scroll down the page to 'eDelivery: Receive your documents in PDF format' ...
- ...and click on 'Enabled'!
Why am I not receiving the PID statement for my co-insured family member in my MyGuichet.lu personal account?
Under the PID scheme, where the CNS pays the statutory contribution directly to the doctor without subsequent reimbursement to the insured person, the PID statement is strictly for the patient's use only.
It should also be noted that, in accordance with the terms of use for MyGuichet.lu, all users must be aged at least 16.
Do I still need to send a document to the CNS?
No! There is no need to send the PID invoice I received from my doctor to my health insurance fund, as payment to the doctor was made directly and no reimbursement is therefore required.
The PID benefits statement I received from the CNS can be sent to my private supplementary insurance provider, if I have one.

Using the CNS's mobile app, GesondheetsApp, I can easily submit my reimbursement claims to my health insurance fund (CNS, CMFEC, CMFEP, EMCFL) digitally.

What are the benefits of Digital Reimbursement?
No more sending claims by post!
With Digital Reimbursement, I no longer need to send my reimbursement claims by post to my health insurance fund.
I pay for the costs upfront during my appointment with my doctor or dentist.
Then, I log in to my personal, secure account on the GesondheetsApp on my smartphone. There, I can view the invoice for the consultation and click on SEND to submit my reimbursement claim to my health insurance fund.
Good to know: I can find information on how to use the GesondheetsApp by clicking on the 'CNS mobile app' tab.
No more waiting!
This digital submission enables my health insurance fund to process a refund quickly.
What if I don't have the GesondheetsApp?
No problem - I'll ask my doctor to print out a paper copy of the invoice.
This paper copy, which I send to my health insurance fund by post, features a special QR code that allows my health insurance fund to quickly retrieve the data (registration number, date of the consultation, fee codes, etc.) without having to enter it manually.
BUT: it's quicker to use the GesondheetsApp. :)
What are the steps?
Step 1
I see my doctor or dentist. The doctor or dentist uses the new Digital Reimbursement system and, at the end of the consultation, I make an upfront payment for the invoice.
Step 2
I choose the option that applies to my situation:
I have the mobile app GesondheetsApp
Step 2
I log in to my GesondheetsApp mobile app.
Under the 'Documents' tab, I can find the invoice for my consultation.
I submit my claim for reimbursement digitally to my health insurance fund by clicking the 'Submit' button.
Step 3
In just a few days, I receive my refund from my health insurance fund.
I don't have the mobile app GesondheetsApp
Step 2
I ask my doctor/dentist to provide me with a printed copy of the invoice featuring a QR code.
I send my claim for reimbursement to my health insurance fund by post.
Step 3
My health insurance fund receives this paper copy and scans it. Thanks to the QR code, my health insurance fund can quickly retrieve the data (identification number, date of the appointment, tariff codes, etc.) without having to enter it manually.
Step 4
A few days later, I receive my reimbursement from my health insurance fund.
BUT: With the GesondheetsApp, it's even quicker 😃
Why use the GesondheetsApp?

With the CNS mobile app GesondheetsApp, I can easily submit my claims for reimbursement to my health insurance fund (CNS, CMFEC, CMFEP, EMCFL) if my doctor or dentist has used Digital Reimbursement as the billing method.
What features does it have?
At present, the only feature of the GesondheetsApp is the digital submission of a reimbursement claim to the health insurance fund.
New features will be made available in due course.
How do I use the app?
The CNS provides a section explaining how to log in to the GesondheetsApp and how to use it.

With Digital Reimbursement - and unlike Immediate Direct Payment (PID) - I make an upfront payment for the invoice.
What sets Digital Reimbursement apart from so-called 'traditional' reimbursement is the ability to submit my reimbursement claim digitally and quickly to my health insurance fund via the GesondheetsApp.
Does the digital claims process apply only to people insured by the CNS?
Digital Reimbursement applies to all insured persons covered by health and maternity insurance in Luxembourg.
Consequently, the GesondheetsApp can be used by insured persons covered by the CNS, the CMFEC, the CMFEP and the EMCFL.
If I use the Digital Reimbursement coverage, what documents will I receive?
FROM MY DOCTOR
I have the GesondheetsApp mobile app
My doctor uploads the invoice to a secure platform. I access the invoice on the GesondheetsApp, then all I have to do is submit my claim for reimbursement by clicking on SUBMIT - no paper is needed anymore!
I don't have the GesondheetsApp mobile app
My doctor provides me with a paper copy of the invoice featuring a special QR code. I send the paper copy to my health insurance fund by post.
FROM THE CNS
Once the CNS has processed the reimbursement, I receive the following from the CNS:
- a reimbursement statement, uploaded digitally to my private eSpace on MyGuichet.lu, if I have activated the eDelivery function;
- a reimbursement statement sent by post, if I have not activated the eDelivery function on MyGuichet.lu.
How do I enable the eDelivery feature on MyGuichet.lu to receive my reimbursement statements digitally?
How do I activate eDelivery? It's very simple:
- Log in to my personal eSpace on MyGuichet.lu using my preferred authentication method (LuxTrust, GouvID, eIDAS) by clicking here.
- Under 'My details > Health insurance fund > Service activation', scroll down the page to 'eDelivery: Receive your documents in PDF format' ...
- ...and click on 'Enabled'!
Do I still need to send a document to the CNS?
Yes! With Digital Reimbursement, I make an upfront payment, then claim a refund from my health insurance fund:
- I have the GesondheetsApp mobile app: I submit my claim for reimbursement using the GesondheetsApp.
- I do NOT have the GesondheetsApp mobile app: I send my claim for reimbursement by post to my health insurance fund.
What should I do if I didn't make the upfront payment at the consultation?
If I did not make an upfront payment at the consultation, the doctor will provide me with a paper copy of the invoice.
Once I have paid, I will send the invoice, together with the bank debit notice, to my health insurance fund by post.
Why isn't Immediate Direct Payment used by default?
Immediate Direct Payment (PID) offers a real advantage for:
- the doctor, as they receive an instant bank transfer from the CNS for the portion of the costs covered by health insurance,
- the insured person, as they only have to pay any excess they may be liable for.
However, PID does not apply to all acts and services provided by doctors and dentists! In such cases, the doctor or dentist must use Digital Reimbursement.
Please note: my doctor is not required to use PID, even if the acts and services carried out during the consultation allow for it! Doctors and dentists may fulfil the digital requirement for invoices either via PID or via Digital Reimbursement.
Are there any circumstances in which I still need to send my application by post, even if I have the GesondheetsApp?
In certain circumstances, despite using the GesondheetsApp, my claim for reimbursement cannot be submitted via the app and must therefore be sent by post:
- Medical prescription to be attached: if the reimbursement requires the submission of a medical prescription, I send my claim for reimbursement by post to my health insurance fund together with the medical prescription.
- Payment not made directly during consultation: if payment of the invoice was not made directly at the time of the consultation, I send my claim for reimbursement, together with proof of payment (bank debit notification), by post to my health insurance fund.
Can I use a different app for digital sending?
Yes!
The MyGuichet.lu mobile app also allows me to submit my claims for reimbursement digitally to my health insurance fund - provided my doctor has selected 'Digital Reimbursement' as the method of coverage.

The GesondheetsApp allows insureds to send their Luxembourg invoices to their health insurance fund from their mobile device in order to claim a refund. For invoices to appear in the GesondheetsApp, the doctor must use the new 'Digital Reimbursement' system.
